What causes knee pain?

Knee pain usually comes from wear-and-tear arthritis (common after 40), injuries to ligaments or the meniscus (common in sports and falls), or overuse. The right treatment depends on the cause. Most people improve with exercise, physiotherapy and medicines, and surgery is for selected cases.

Most common: osteoarthritis, ligament and meniscus injuries, overuse.
Diagnosis: examination and X-ray, MRI when an injury is suspected.
Goal: less pain, stronger muscles and safe, confident movement.
Common causes

What could be causing
your knee pain?

Where it hurts, how it started and your age all help point to the cause.

Osteoarthritis

Gradual pain and stiffness, worse on stairs and after rest. Common after 40.

Ligament injury (ACL)

A twist or "pop" during sport, then swelling and a knee that gives way.

Meniscus tear

Pain on twisting, clicking or the knee getting "stuck".

Runner's knee

Ache around the kneecap from running, squatting or stairs.

Tendinitis

Pain just below or above the kneecap after activity.

Gout or inflammatory arthritis

Sudden hot, red, swollen joint, sometimes in several joints.

Bursitis

A soft, swollen bump on the front of the knee from kneeling.

Fracture

After a fall or accident, with severe pain and trouble standing.

Should you see an orthopaedic doctor? Tick what applies.

This isn't a diagnosis. It helps you decide between home care and seeing a specialist.

Don’t walk it off if you heard a pop with immediate swelling, can’t straighten the knee, or the knee looks deformed after an injury. Get it checked the same day.
Why not ignore it

Long-standing knee pain tends to snowball.

Avoiding movement weakens the muscles that protect the knee, and that brings more pain. Untreated injuries can also damage the joint over time.

The good news: with the right exercises and treatment, most people get back to walking, stairs and sport comfortably.
  • Faster wear of the joint cartilage
  • Weak thigh muscles and poor balance
  • Higher risk of falls
  • Overuse of painkillers, which can affect the stomach and kidneys
  • Weight gain from reduced activity
How we find the cause

A clear diagnosis
before any treatment.

Most people need only an examination and an X-ray. Further tests are added only if they change the treatment.

  1. Consultation

How the pain started, what makes it worse, your activity level and goals.

  2. Examination

Movement, swelling, stability and alignment of the knee, compared with the other side.

  3. X-ray

Shows arthritis, alignment and bone problems, done in-hospital.

  4. MRI or blood tests if needed

MRI for ligament or meniscus injuries, and blood tests for gout or inflammatory arthritis.

Hurt your knee playing sport? See a doctor within a few days if it swelled quickly or gives way. Early diagnosis of ligament and meniscus injuries gives the best results.

knee pain treatment.

Ask a question
When should I see a doctor for knee pain?

See an orthopaedic doctor if pain lasts more than 2 weeks, the knee swells, locks or gives way, or pain started after an injury. Go the same day if you can't bear weight or the knee is hot and swollen with fever.

Can knee pain be treated without surgery?

Yes. Most knee pain improves with physiotherapy, strengthening exercises, weight management and medicines. Surgery is advised only for specific injuries or severe arthritis.

What is the best exercise for knee pain?

Strengthening the thigh (quadriceps) and hip muscles helps most. Walking, cycling and swimming are also good. Your doctor or physiotherapist will show you exercises suited to your knee.

Do I need an MRI for knee pain?

Not always. An X-ray is enough for arthritis. An MRI is advised when a ligament or meniscus injury is suspected, or if the diagnosis is unclear.

How long does recovery take after arthroscopy?

Simple procedures may allow walking within days. Ligament reconstruction usually needs several months of rehabilitation before returning to sport.

Is knee pain at a young age serious?

It's often due to overuse or sports injury and responds well to treatment. Get a knee that swells or gives way checked early.
